The study (survey + semi-structured follow up interviews) was approved on 2/26/09 as study #0902000069 by the Indiana University Bloomington Institutional Review Board. Technically the study was approved with exempt status without the requirement of a study information sheet, which is they the information sheet of the survey does not contain all of the usual sections and wording required of an expedited of full review study.
